What To Expect

  • Free consultation at client's convenience (by phone, or in person).
  • Tender Touch is available 7 days a week.
  • Medical needs of the client are evaluated by personally reviewing medical records.
  • Tender Touch communicates with any healthcare provider that would enable better identification of all medical needs of the client.
  • Determination is made about what level of care and what type of facility would be most appropriate.
  • The client’s financial situation is reviewed to determine what is affordable. Will the client be able to private pay, or does the client need to apply for state assistance?
  • Appropriate sources are provided to assist with financial planning, VA benefits, or ALTCS process.
  • Geographic area most convenient for the family is learned.
  • Tender Touch will present options to the client and family that meet their medical, social, location and financial requirements.
  • Support is provided to the family throughout their decision making process.
  • Tender Touch will provide personal tours of potential facilities.
  • Coordination is provided with Social Workers and RN Case Managers to arrange services and equipment for client’s discharge and transfer to the chosen facility.
  • Tender Touch will arrange transportation for the day of discharge.
  • Tender Touch provides personal follow-up in homes and facilities after placement.
